Introduction
Synergy Meridian Clinic ("the clinic," "we," "our") operates a Traditional Chinese Medicine practice at Unit 4-B, Perfecto Building, 142 Legazpi Street, Legazpi Village, Makati City. We take patient confidentiality seriously both because Philippine law requires it and because four generations of our family have treated it as a professional obligation. This policy explains what personal information we collect, why we collect it, how we store and protect it, and what rights you have as a patient and visitor.
Information We Collect
When you contact us through the website, by phone, or in person, we may collect the following:
- Your full name, as required for clinic records and HMO direct-billing.
- Your email address, when you submit a contact form or book a consultation.
- Your phone number, to confirm appointments and send reminders.
- Health notes that you share voluntarily — current symptoms, relevant medical history, current medications, allergies, pregnancy status, and any information you disclose during consultation. Clinical notes from your visits are recorded in our patient records.
- HMO membership details when you request direct-billing through Maxicare, Medicard, Intellicare, or another accredited network.
- Basic technical data when you visit the website (IP address, browser type, pages visited) for security and diagnostic purposes.
We do not ask for information we do not need, and we do not purchase or aggregate patient data from third parties.
How We Use Your Information
Your information is used only for the following clinical and administrative purposes:
- To confirm, schedule, and reschedule consultations.
- To conduct a safe and accurate clinical assessment — pulse and tongue diagnosis, pattern identification, herbal prescription.
- To process HMO direct-billing and issue official receipts.
- To contact you with treatment-relevant information between visits.
- To maintain the clinical record required under Philippine Department of Health regulations for traditional and alternative health care facilities (DOH AO 2018-0017).
We do not use your information for marketing by third parties. We do not use your health information for any purpose not directly related to your care.
Data Storage & Security
Electronic records are held on access-restricted servers with encryption at rest and in transit. Only the physicians, the senior herbalist, and one administrative staff member have access to identifiable clinical records, and each access event is logged. Our internal safeguards are designed to meet the reasonable technical and organisational standards expected of a HIPAA-equivalent clinical practice, adapted for Philippine regulatory requirements.
Physical records — handwritten pulse-and-tongue sheets, herbal prescription carbons, and intake forms — are kept in a locked cabinet within the clinic premises. Physical records older than seven years are shredded on a documented quarterly schedule.
Sharing of Information
We do not sell, rent, or share your personal or health information with third parties for commercial purposes. We will share limited, necessary information only in the following circumstances:
- With your accredited HMO provider, strictly for the purpose of direct-billing, and only with your explicit consent at the time of booking.
- With a referring or co-managing clinician (such as your rheumatologist, orthopaedic surgeon, or physiotherapist), only with your explicit written consent.
- Where required by Philippine law, including lawful requests from the National Privacy Commission, the Department of Health, or a Philippine court.
Cookies
The website uses minimal cookies — essentially, a single session cookie to remember if you have dismissed the cookie notice, and standard technical cookies for site functionality. We do not use advertising trackers, social media pixels, or cross-site behavioural analytics on pages that handle health information.
Your Rights under RA 10173
Under the Philippine Data Privacy Act of 2012 (Republic Act 10173), you have the right to:
- Be informed about how your personal data is processed.
- Access the personal data we hold about you.
- Rectify inaccurate or outdated information.
- Object to processing and withdraw consent, subject to clinical and legal record-keeping requirements.
- Erase or block information that is no longer necessary, has been unlawfully processed, or that you have validly withdrawn consent for.
- Data portability — request a copy of your records in a commonly used format for transfer to another provider.
- File a complaint with the National Privacy Commission if you believe your rights have been violated.

Synergy Meridian Clinic is registered as a Personal Information Controller with the National Privacy Commission of the Philippines.
